Blastomycosis is a dangerous infectious disease, which, fortunately, is not found in our country. Its typical appearance is in the states of North and Central America.
Reasons
The cause of infection is parasitic fungi from the group of blastomycetes. These fungi live in the soil, rotten wood. Mold can get into the air along with dust and other particles and it can be inhaled. People with weakened immune systems have a much higher risk of infection . A higher incidence of infections is observed in the spring and autumn months, when humidity increases.
Manifestations
A typical blastomycosis occurs under the guise of a lower respiratory tract infection, that is, pneumonia. The infection manifests itself by coughing, shortness of breath and an increase in body temperature, does not respond to treatment and may eventually spread to other organs and tissues. It is worth mentioning the damage to the kidneys, brain, prostate (painful inflammation of the prostate), bones (bone pain) and skin, where a rash with ulcers can develop. Severe cases occur with the image of lung shock ( ARDS ) and they can kill the patient relatively quickly.
In chronic blastomycosis, symptoms that mimic tuberculosis manifest themselves in the form of prolonged fever, night sweating, fatigue and chronic cough.
Diagnostics
The diagnosis can be considered in case of clinical difficulties in a patient who is (or has recently been) in the zone of blastomycosis. If the patient has a confirmed immune disorder, the suspicion is even greater. Pneumonia can be seen on an X - ray of the chest or on a CT scan , and the absence of a reaction to antibiotics is also assumed . Confirmation of the presence of blastomycetes can be performed serologically, while specific antibodies against blastomycetes are detected in the blood. As part of the study of chronic blastomycosis, it is certainly advisable to exclude tuberculosis using the Quantiferon test.
Treatment
The basis of treatment is the so-called antifungal drugs. Acute phase therapy includes, in addition to intravenous antifungal drugs, oxygen therapy and cough medications. If pulmonary insufficiency occurs , intubation and artificial ventilation of the lungs may be required .
